*Mill Name or Code: The mill code is a 4 digit number.
*Style and Color
*Width and Length
*Carpet Edging: Other Edging (Describe in comments)
Bound to match primary color
Serged to match primary color
Options: Unless below options are selected, (1) carpet will run the length of the broadloom roll (2) a pad will not be included and (3) standard delivery to receiving warehouse only is incluided: Add a pad (recommended).
Quarter-turn carpet (if possible) to reduce cost and waste.
Request residential delivery and spread
Notes Add notes here, including description of project, binding type and color, special requests, etc.
